NORTHWEST INDIANA | Powers Health is making it easier for patients to manage chronic heart conditions with the launch of its new Advanced Heart Failure Services. This program gives patients expert, ongoing care in an easy to access outpatient setting and is especially helpful for people with complex heart failure.
Living with heart failure can feel overwhelming. The goal of this new program is to take the burden off families by providing focused, ongoing medical care that keeps symptoms under control without the need for overnight hospital stays.
Conditions managed include:
- Worsening heart failure
- Patients transitioning from inpatient to outpatient care
- Recurrent hospitalizations for congestive heart failure
- Complex medication and volume management

Located on the second floor of Community Hospital, 901 MacArthur Blvd., the clinic operates from 8 a.m. to 4 p.m. Monday through Friday. Patients must obtain a medical referral.
